The Complex Rehabilitation Midlands Case Management Team (NHS England – Midlands), have developed this Community Directory database to support services, when discharge planning patients requiring neurological rehabilitation.

We aim to update the information on a six monthly basis. However, if you notice discrepancies in the information provided, or would like something updating or a new service adding, then please contact the team on england.wm-neurorehab@nhs.net and we will follow this up.
